Methodist Healthcare is currently in the process of building out a retail pharmacy on the ground floor of Methodist Hospital, located at 7700 Floyd Curl Dr., San Antonio, TX 78229. Based on discussions with the Texas State Board of Pharmacy, the retail pharmacy requires the assignment of a suite number in order to facilitate issuance of its pharmacy license.
Attached for your review are our completed Request for Address – Assignment/Verification form and the applicable life safety drawings identifying the location of the retail pharmacy suite.
Please note: Our request is limited to the addition of an Accessory Address for the retail pharmacy. No other changes or amendments to the existing Methodist Hospital campus address are requested.
